Billing & Insurance

We participate with most local and many national insurance plans. However, should you have any questions pertaining to our providers being in network you may visit your insurance company online or call our office for verification. It is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.

Please contact your insurance carrier or our billing office at (580) 531-0022, option 1 for insurance or option 2 for billing should you have any additional questions.

Please be aware that in order to determine the correct diagnosis so that the most appropriate treatment can be recommended, your provider may recommend or perform certain in-office procedures that aid in diagnosis. If performed, these procedures are billed as a separate item on your office visit statement. These diagnostic procedures are often classified as “surgical procedures” by your insurance company and may incur a deductible. The most common diagnostic procedures which may fall under your surgical deductible include: endoscopy, laryngoscopy, debridement and cerumen removal.
